What does “en efectivo” mean in Spanish?

En efectivo means “in cash” — paying with actual bills and coins instead of a card. Dinero en efectivo is “cash money.” You will hear it at shops and restaurants: ¿Paga en efectivo o con tarjeta? (“Are you paying in cash or by card?”). The word efectivo on its own can also mean “effective,” but with money it always means cash.
